Click “ship” and your patient will receive their device either in their home or at your office; you choose.

Review data from your patient's device in the Prevounce portal or your integrated EMR. Staff are automatically notified if a patient’s readings enter a specified caution or critical range, and reports can be customized to best suit your workflows. Finally, if you are logging time toward RPM management billing codes, Prevounce automatically tracks that too!

Get paid. The Prevounce billing system generates a report every month clearly showing all billable RPM codes for each patient. Fees are charged only for billable RPM patients and include the connected device, cellular service, hosting and customer support. You simply submit your claims at the start of each month; fees aren’t due until after you receive your Medicare payment.

For congestive heart failure patients, sudden weight gain is often the first sign that their condition is worsening or that adjustments to medication should be made, for example. Remote monitoring can increase the chance of early detection, allowing for intervention before hospitalization or critical care are needed.

Diabetes is one of the disease areas with the highest awareness of digital health amongst patients, and it’s also one of the most effective RPM applications. Remote monitoring of glucose levels reduces doctor visits for patients, making care of an ailment that changes not just daily but hourly more timely and efficient. RPM also offers significant benefits to patients’ A1C levels. In one study, 70 percent of high risk diabetic patients were able to lower their A1C levels with use of an RPM device.

Frequently Asked Questions

Who can perform Remote Patient Monitoring (RPM) under CPT Code 99457?

All practitioners must practice in accordance with applicable state law and scope of practice laws, but CPT Code 99457 allows reimbursement for time spent by the billing physician, a qualified healthcare professional (“QHCP”), or clinical staff.

How is Remote Patient Monitoring different from Telehealth?

In short, reimbursement for RPM services is not limited by geography – to rural or medically underserved areas – and there isn’t any “originating site” requirement for RPM services. They can be provided wherever a patient is located.

Are RPM services (CPT Codes 99453, 99454, 99457 and 99458) only reimbursable for patients with multiple chronic conditions?

No. A practitioner can recommend RPM services for any patient that may benefit from some form of remote patient monitoring.

Is CPT Code 99454 reimbursed per device or per patient?

CPT Code 99454 can only be billed once per patient each 30 days, whether the patient is using one device or more than one.

Can RPM Also Be Billed with Chronic Care Management (CCM)?

Yes, a provider can bill both CPT 99457 and CPT 99490 in the same month. This is allowed because CMS recognizes the kind of analysis involved in furnishing RPM services is complementary to CCM and other care management services. However, time spent furnishing these services cannot be counted towards the required time for both RPM and CCM codes for a single month (i.e., no double counting). Accordingly, billing both requires at least 40 minutes total (20 minutes of CCM and 20 minutes of RPM).
