Enroll patients

Our team helps you recruit, enroll, and capture consent from patients. For CCM, initiating visits are required for patients you haven’t seen in the past 12 months. RPM patients must be established prior to enrollment.

Customize care plans

We develop custom patient care plans using evidence-based guidelines under in-house physician supervision. All you need to do is review and approve.

Manage care

Our care managers call patients, document in practice-accessible patient profiles on the Prevounce platform, and log time toward billing thresholds. We proactively alert providers to patient issues through their preferred communication channels.
