Recognizing Sandra’s distress and the urgency of her symptoms, her care manager took immediate action. The care manager initiated a three-way call with the patient and the clinic directly. Through that extra effort and advocacy, they were able to secure a same-day appointment for Sandra.

A Potential Crisis Averted

During the same-day appointment, Sandra was evaluated and treated, receiving multiple tests that day, one of which revealed a severe infection that she wasn’t aware of. Catching this infection early prevented further complications and allowed for a targeted treatment and recovery plan. 

Ongoing Care and Support

After this episode of care, Sandra expressed sincere appreciation for her care manager’s check-ins and support. A strong rapport built on consistent communication has been established between Sandra and her care manager.
