Recognizing the potential medical emergency, the care manager acted without hesitation, coordinating with emergency medical services and the local fire department to ensure Melissa received immediate attention. The care manager stayed involved throughout the emergency, offering support and reassurance while EMS arrived.

Safe Transport to the Hospital for Urgent Care

Thanks to the care manager’s swift decision-making and calm approach, Melissa was safely transported to the hospital for urgent care. The timely intervention likely prevented further complications from a hypertensive crisis. In addition to facilitating emergency care, the care manager thoroughly documented the encounter and followed up directly with Melissa’s provider to ensure continuity of care and post-hospital support.
