SMS and Communication Terms and Conditions

Prevounce Health ("Prevounce," "we," "us," or "our") provides Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) technology services to healthcare providers. By enrolling in these care programs and providing your mobile phone number, you agree to these SMS Terms of Service.

1. Program Description

The Prevounce SMS service allows your healthcare provider (your "Practice" or "Doctor") to communicate with you regarding your prescribed care plan. Prevounce facilitates these messages on behalf of your provider.

Message content may include:

  • Device Logistics: Notifications regarding the shipment, delivery, and setup of medical devices (e.g., blood pressure cuffs, scales).
  • Clinical Alerts: Two-way chat with your assigned Care Manager regarding your physiological readings (e.g., "Your blood pressure reading was high, let's discuss").
  • Scheduling: Reminders for required monthly check-in calls or clinical appointments.
  • Support: Troubleshooting assistance for your connected devices.

2. Message Frequency

Message frequency will vary depending on your specific care plan, the number of devices you use, and your engagement with your care team. This is not a fixed-schedule marketing campaign; messages are sent based on clinical necessity.

3. Cost and Carriers

Message and data rates may apply to each message sent or received in connection with this program, as provided in your mobile telephone service rate plan.

Important: Carriers (including T-Mobile, Verizon, AT&T, and others) are not liable for delayed or undelivered messages.

4. Eligibility

To receive SMS messages from Prevounce, you must be a resident of the United States and 18 years of age or older. By enrolling, you represent that you are the account holder for the mobile number provided.

5. User Opt-Out (STOP)

You may cancel this SMS service at any time. To stop receiving messages, reply STOP to any message you receive.

After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from this program. If you want to join again, you will need to contact your healthcare provider or reply START or UNSTOP.
