As part of the practice's commitment to quality care, it had initially implemented a remote care management program for its Medicare patients. However, the program — which included chronic care management (CCM) and remote patient monitoring (RPM) services — struggled due to the approach of the practice's previous vendor. The vendor's model focused heavily on repetitive, scripted phone calls that patients found disengaging. Clinicians also felt the outreach had become a "check-the-box" exercise to meet monthly billing requirements rather than something that provided meaningful clinical value.

Following extensive discussions with Prevounce, the practice relaunched its remote care management program with APCM and RPM services. These insights have allowed for immediate clinical interventions that prevented life-threatening events.
