Marion Health, a 100+ bed independent hospital system based in Marion, Indiana serving a rural community, faced significant challenges in monitoring high-risk patients. Marion’s team previously had community health workers collecting vital signs from patients’ homes, but they were not receiving reimbursement for this labor-intensive approach. While the organization had experimented with other remote patient monitoring (RPM) device companies, unreliable support and lack of connectivity in rural areas led to limited and unsatisfactory program success.

The partnership began with on-site collaboration between Prevounce and Marion Health’s care team. Prevounce facilitated a seamless RPM program launch, working the program into the hospital’s workflows and integrating with the hospital’s eClinicalWorks EHR system. 

With a grant-funded purchase of over 500 custom-branded RPM devices, Marion Health deployed Pylo cellular-connected weight scales, blood pressure monitors, pulse oximeters, and glucose meters. The devices from Pylo, a Prevounce company, proved especially effective in overcoming prior connectivity issues, enabling consistent and accurate data transmission.

 

Addressing Cardiovascular Health

 

The RPM program launched in Marion Health’s Chronic Care Clinic, where patients receive remote monitoring for conditions like hypertension and heart failure. Some of these patients were equipped with multiple different devices to better ensure comprehensive monitoring of their complex cardiac conditions. The hospital quickly saw improvements in its patients’ blood pressure control and overall cardiac health outcomes.

 

Expanding Into Maternal Health

 

Following the success with cardiac patients, Marion Health expanded its RPM program to maternal health. Given the severity of maternal health risks in the hospital’s community, RPM was introduced to support expectant and new mothers with conditions like gestational hypertension and gestational diabetes. Since RPM’s implementation, Marion Health has observed better health outcomes for mothers and their newborns. 

Due to the success of its cardiovascular and maternal health RPM programs, Marion Health has begun to expand remote patient monitoring into other specialties and conditions.

"While working with our other RPM vendors, we struggled with timely responses from the help desk, especially when calling from a patient's home," Collins says. "My staff have stated this doesn't happen now."
