A 200+ bed health system employing over 1,500 healthcare professionals in Upstate New York sought to improve patient outcomes and increase its revenue through remote care management. The health system found and selected Prevounce for its proven solutions to deliver comprehensive remote care management services including remote patient monitoring (RPM) and chronic care management (CCM). Key to this decision was Prevounce’s expertise in integrating its platform with Epic.
Before Prevounce, the health system was in the process of transitioning to Epic from a previous EHR. They were eager to implement an RPM and CCM program, but were wary of it interfering with their major EHR transition. Prevounce worked closely with the hospital’s team to create a project plan and develop an Epic integration that would not only give them seamless access to RPM and CCM data directly within the EHR, but also not contribute to the burden of switching systems. This integration was thoroughly tested and ready for deployment with the Epic go-live, ensuring a successful remote care program launch with minimal disruption to clinical workflows and improved continuity of care.
With limited internal staffing options, the health system opted to use Prevounce’s outsourced care management services rather than manage their RPM and CCM program in-house. Prevounce’s care management team has enabled the health system to achieve a high patient compliance rate, with almost every patient enrolled in RPM and CCM consistently billable each month. Patients benefit from an average of 20 minutes per month of individualized time with a Prevounce care manager. Care managers actively monitor RPM device readings, and escalate noteworthy changes to the providers as needed. Providers have noted that receiving this data and being able to make medication titration and adjustment decisions remotely has been incredibly helpful—both for enhancing patient care and improving clinical efficiency.
The health system initially piloted the RPM and CCM program in its residency clinic as a proof of concept. Following the success of the pilot, the program has since expanded into the health system’s women’s health clinics. Driven by improved patient outcomes and increased revenue, the health system is in the process of expanding the remote care management program into additional departments.
